By completing this application, I HEREBY AGREE to support and abide by ALL rules, policies, objectives and interests of the Bradford West Gwillimbury Minor Hockey Association.  I am aware that the initial objective of minor sports focuses on the personal and character development of each individual participant.  I will therefore conduct myself in an appropriate and responsible manner, at all times, and be fair to all participants, in all aspects of the position that I am applying for.  I am aware and agree that any behavior on my part that would be contrary to the BWGMHA Rules and Regulations could forfeit my position at any time.

**NOTE – OMHA requires all centers complete a police record check on all carded officials for the 2010/2011 season.
