By completing this application, I HEREBY AGREE to totally support and abide by ALL THE RULES, objectives and interests of the Bradford West Gwillimbury Minor Hockey Association.  I am aware that the first aim of minor sports is the personal and character development of each individual participant.  I will therefore conduct myself in a proper and responsible manner at all times and be fair to all participants in all aspects of the position that I am applying for.  I am aware and agree that any behavior on my part that would be contrary to the BWGMHA Rules and Regulations could forfeit my position at any time.

**NOTE – OMHA requires all centers to do a police record check on all carded officials for the 2010/2011 season.
