By completing this application, I HEREBY AGREE to totally support and abide by ALL THE RULES, objectives and interests of the Bradford West Gwillimbury Minor Hockey Association.  I am aware that the first aim of minor sports is the personal and character development of each individual participant.  I will therefore conduct myself in a proper and responsible manner at all times and be fair to all participants in all aspects of the position that I am applying for.  I am aware and agree that any behavior on my part that would be contrary to the BWGMHA Rules and Regulations could forfeit my position at any time.

BWGMHA reserves the right to select particular coaches from the submitted applications for the interview process.

 

**NOTE – OMHA requires all centers to do a police record check on all carded officials for the 2012/2013 season.

BWGMHA reserves the right to contact only selected individuals for interviews.
