Instructions:

Use the drop down boxes to select your response for each question.  Please answer each question honestly reflecting your satisfaction level. You may also make additional comments in the text boxes below each staff member.

Once the form is completed, you may submit it online or print it out and mail it directly to the Bradford West Gwillimbury Hockey Association at the address below. If you prefer, you may also hand it to your team manager who will ensure that it gets to the Technical Director.
